Characterization and surgical staging of a solid, enhancing renal mass.

Applies to

Solid enhancing renal mass on contrast-enhanced CT or MRI

Does not apply to

Predominantly cystic masses (use Bosniak)
Suspected infection, infarct, or lymphoma in the appropriate clinical context

What to report

1.Size and laterality
Maximum diameter (drives T stage) and side.
2.Enhancement
Degree and pattern of enhancement across phases; report unenhanced and post-contrast HU.
3.Macroscopic fat
Macroscopic fat suggests angiomyolipoma.
Pitfall: A fat-poor AML cannot be distinguished from RCC on imaging — do not exclude malignancy on that basis.
4.Margins and infiltration
Well-defined versus infiltrative; perinephric fat stranding or fascial involvement.
5.Venous extension
Renal vein and IVC tumor thrombus, including cranial extent relative to the diaphragm.
6.Nodes and metastases
Retroperitoneal nodes, adrenal, and distant metastases for staging.
7.Nephrometry descriptors
Record RENAL components plus the anterior/posterior (a/p/x) and hilar (h) descriptors for the surgeon.
