1.Maximum stenosis
Grade the highest-grade luminal diameter stenosis in any vessel ≥ 1.5 mm — this sets the CAD-RADS category.
Pitfall: Dense calcification causes blooming and can overestimate stenosis; state diagnostic confidence.
2.Extent
Number of vessels with severe (70–99%) disease, and any left main involvement — these separate 4A from 4B.
3.Plaque burden (P)
Overall plaque burden (segment involvement/CAC) as the P modifier; higher burden carries higher event risk.
4.High-risk plaque (HRP)
Low-attenuation plaque, positive remodeling, spotty calcification, napkin-ring sign.
5.Modifiers
N (nondiagnostic), S (stent), G (graft), I (ischemia by CT-FFR/CTP), E (exceptions/non-atherosclerotic).
